Optage® In-home Care
Optage® In-home Care extends the high quality care of Presbyterian Homes & Services into the homes of older adults. Personalized care plans are designed to deliver just the services needed by an individual to build a bridge to optimum aging. The highly trained nurse-managers and caregivers of Optage® In-home Care also provide coordination of care and communication to contribute to the health and safety of older adults who wish to remain in their homes.
Optage® In-home Care is Medicare-certified for services provided in Minnesota.
Optage® Hospice
Optage® Hospice recognizes that every person deserves to live out their life with respect and dignity, alert and free of pain, in an environment that promotes quality of life.
When treatments are no longer successful or additional support or pain management is needed, hospice may be the right solution. Hospice provides compassionate care that focuses on comfort, quality and support for individuals with life-limiting illness, their families and caregivers. Services are provided in surroundings that are familiar; a private home, assisted living, memory care, skilled nursing care center – wherever is home.
Hospice is covered by Medicare, Medicaid, HMO, private insurance, self-pay and charitable donations. Optage® Hospice is Medicare-certified.
Optage® House Calls
Optage® House Calls provides comprehensive care to older adults at their place of residence. A personal physician and nurse practitioner, with extensive experience in geriatric care, lead a team of individuals who collectively provide personal and coordinated care. This special kind of comprehensive care responds to your needs and preferences.
The House Calls team assesses your needs in order to identify what is necessary to maintain the highest possible level of comfort and independence.
We develop and continuously update your individualized plan of care. We pay attention to details, provide timely visits, and anticipate needs. House Calls ensures you receive the care and attention you deserve.
